HIPAA NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
1. OUR COMMITMENT
Ironside Home Health LLC is committed to protecting the confidentiality of your protected health information (PHI).
2. USES AND DISCLOSURES
We may use and disclose your PHI for:
- TREATMENT: To provide, coordinate, or manage your health care
- PAYMENT: To bill insurance companies (TRICARE, Medi-Cal, Medicare, private insurance) or you directly
- HEALTH CARE OPERATIONS: For quality assessment, staff training, licensing, and administrative activities
3. OTHER PERMITTED USES
Without authorization, we may disclose PHI to comply with law, for public health, to report abuse or neglect, for health oversight, for court orders, for law enforcement, to coroners, for organ donation, for research, to prevent serious threats, or for workers compensation.
4. USES REQUIRING WRITTEN AUTHORIZATION
Most other uses require your written authorization, including marketing communications, sale of PHI, and psychotherapy notes. You may revoke authorization in writing at any time.
5. YOUR RIGHTS
You have the right to:
- Access and copy your PHI
- Request amendments to your PHI
- Receive an accounting of disclosures
- Request restrictions on uses or disclosures
- Request confidential communications
- Receive a paper copy of this Notice
- Be notified of a breach affecting your PHI
6. OUR RESPONSIBILITIES
We are required by law to maintain the privacy and security of your PHI, provide this Notice, follow the terms of this Notice, and notify you of any breach of your unsecured PHI.
7. CHANGES TO THIS NOTICE
We reserve the right to change this Notice. Updated notices will be available at our office and on our website.
